IMMEDIATE SUPERVISOR: Program Director
Crisis Stabilization Support Specialist, services consumers who are experiencing acute and severe mental health conditions during short-term admission in the crisis stabilization program. Due to the acute nature of symptoms and short admissions in the program and short time periods to link individuals with step down services and resources, service coordination and safety planning are essential skills for Qualified Mental Health Professionals. The services are provided within the community setting. Must be registered as a QMHP-A with the Board of Counseling Position reports directly to Program Director.
DUTIES:
Establish open lines of communication for individuals with mental/behavioral/emotional issues
Provide direct, Crisis Stabilization Support Services to identified adults
Implement treatment goals and objectives
Facilitate therapeutic sessions for clients and their family for immediate/preventive needs
Complete Crisis education, prevention planning, progress notes and discharge summaries
Acts as an advocate on behalf of the client and their family in relation to service needs
Facilitate referrals to other healthcare or counseling professionals
Collaborate with additional treatment providers and placement agencies
Attend mandatory trainings and supervisions
Act as the liaison between family, referring agencies, and other community organizations
Coordinate consumer services between various agencies
Adhere to agency’s documentation policies
Maintain compliant case records and accurately document services rendered
Attend staff meetings consistently. Prior approval from management team is required when absent
Maintain high degree of consumer confidentiality and professional boundaries
Follow the agency’s crisis protocol
Other duties as required by the needs of the company.
QUALIFICATIONS:
Bachelor’s degree in human services or a related field from an accredited college.
Registered with the Board of Counseling as a QMHP-A days of employment.
