Mental Health Support Specialist

IMMEDIATE SUPERVISOR: Supervisor
The QMHP-C will provide intensive in-home mental health services that are goal-directed, structured and supportive to individuals at risk for removal from the home and their families.
DUTIES:
Provide family focused intensive in-home counseling for client and family
Reduce the risk of out of home placement
Assist with strategies and interventions in developing coping skills. 24 hour crisis intervention
Attend all annual mandatory agency and staff training (CPI, CPR First Aid, Human Rights, HIPPA
Submit all progress, reimbursement and accountability documentation weekly
Complete ISP, Quarterlies
Provide written documentations of individual and family sessions.
Participates in the person centered treatment plan within the initial thirty-day period
Monitor and assess client/family situations
Monitor consumer services and review discharge status as needed
Engage referral agency as needed
Have weekly contact with the Referring Agency
Attend case supervision with supervisor
Act as the liaison between family, referring agencies, and other community organizations
Coordinate consumer services between various agencies
Adhere to agency’s documentation policies
Maintain compliant case records and accurately document services rendered
Maintain high degree of consumer confidentiality and professional boundaries
Prior to the end of each ninety-day service period, assist to coordinate a staffing for the consumer
Follow the agency’s crisis protocol
Participate in person center plan meetings at consumers’ request
Other duties as required by the needs of the company.
REQUIREMENTS:
Monthly supervision and training

Report writing and documentation
Willingness to work in stressful situations
QUALIFICATIONS:
Bachelor’s degree in human services or a related field from an accredited college.
Registered with the Board of Counseling as a QMHP-C in accordance with 18VAC115-80 to provide collaborative mental health services for children or adolescents.
